First, you are missing Jeremy Downs, the teenager that tried to kill another guy in first season, telling that he was going to show him a crocodile. Due to Dexter, this guy kills himself, so it would be a NoCode indirect kill, I think.
Finally, you can consider Joseph Driscoll as an indirect kill too, as he was probably murdered because he was Dexter's father, so that was a possibility for the ITK to make Dexter go to his father's house
The only thing I think you are missing is that there are /two/ men that I can think of that James Doakes killed, and they don't have little markers next to them. One was the black man walking on the street that James chased, and Batista followed in the car. Was found dead under the bridge and Doakes kept saying that it was self defense. The other incident was when him and a former SWAT (was it SWAT?) were on the boat together, and the other former kept telling him that he was gonna go Cuba and wanted Doakes to go to. The former swat also killed his wife and admitted it to Doakes on the boat. Both of those were in season 2 with Lila. Don't remember what order they were in though.
